Applicants for the 2018 RYLA Youth Leadership Academy will be asked to fill out and submit numerous forms before they can attend the three day summer program.
 
The first step for applicants will be to complete RYLA Form #1 — "Parental Consent Form" and submit using the Submit button at the bottom of the application.

The second step for applicants will be to complete RYLA Form #2 — "Candidate Application" — and submit it using using the Submit button at the bottom of the application.
 
Before filling out this form, we recommend you view it first to determine whether you have all the necessary information at hand to complete all the required fields. You will not be able to save a partially completed form.

The third step in the candidate application process will be to complete and submit RYLA Form #3 — "Leadership Essay form"
 
This form will be evaluated to help prepare you for the Rotary Youth Leadership Academy. It should include your definition of leadership and how you currently and/or in the future see yourself fitting that definition.
 
The form asks you to complete three statements:
 
  • Leadership is...
  • My present leadership activities include...
  • My vision of my leadership role is...
 
We recommend that you prepare your answers to each question in advance and then copy and paste them into the proper text areas of the form. The maximum number of characters (not words) for each answer is 5,000.

RYLA Form #4 — "Code of Conduct" — should be read carefully, completed and submitted by the RYLA program participant. This form outlines a mandatory code of conduct that every participant agrees to follow during the Rotary Youth Leadership Academy session.
 

 
RYLA  Form #5 — " Release of Liability, Permission for Medical Care and Emergency Information" — needs to be completed and submitted by the parent(s) or guardian(s) of each program participant. The RYLA Program involves a variety of activities that often include games, group initiative problems, low and high ropes course elements, other physical activities and living in a residence hall. Attendance at all events is required while the level of participation is at all times completely up to the individual's choice. Yet there is a risk, which must be assumed by each participant that he or she may suffer an emotional or physical injury. Policy for participation in this program requires that a parent or guardian of every participant complete this form. Certain health medical information must be made known to the leaders conducting the course so they are prepared to respond appropriately if the need arises. This information will be held in confidence.

RYLA Form #6 — Release for Use of Photos/Videos — is a standard release provided by both the RYLA program participant and his/her parent or guardian allowing RYLA program representatives to take photos and videos of program participants and to publish, circulate or broadcast those photos or videos for any purpose deemed proper.
